Terms and conditions

Consultation Terms, Conditions and Warnings

This information is very important. It explains the risks you or your child (“you”) are assuming by arranging a consultation with a consultant trading as Wise Nutrition (trading under Intouch Chirocare Ltd) (“Practitioner”). It is very important that you read and understand it completely as Wise Nutrition and it's Practitioners intend to rely on you having done so.

Your Practitioner is Joanne Wise, your contract is with Intouch Chirocare Ltd, trading as Wise Nutrition, a company registered under number  SC288270, whose registered address is Summit House, 4-5 Mitchell Street, Edinburgh, Scotland, EH6 7BD . 

Consultations with your Practitioner are subject to these terms, conditions and warnings.

Warning

The nutritional and lifestyle advice we may give you is based on the information you give us. Any nutritional and lifestyle guidelines given are meant only for you, and we expressly disclaim responsibility for any third party use of our strategies or plans. It is your responsibility to provide complete and accurate information. Any misinformation or omitted information may affect the assessment and advice. Any incomplete or misrepresented information is solely your responsibility and we expressly disclaim responsibility for any resulting consequences.

Your GP/Doctor

We provide nutritional and lifestyle consulting, which are no substitute for proper medical advice. We are not licensed to diagnose a medical condition or illness. You must consult a medical doctor for any health concerns or medical advice.

If you are receiving treatment from a doctor or any other medical practitioner, you should inform them of any nutritional strategy, because of a potential reaction between the nutrition and the medication.

Health history

Our ability to recommend appropriate solutions is greatly improved by having more background information to work on. So please ensure the health questionnaire is filled out as completely and accurately as possible.

You should provide us with any medical diagnosis, medication, or other treatments you are taking, as this could affect the assessment and advice.

Recommended products and services

We may recommend testing and/or food supplements, herbal supplements and other products as part of your health strategy. You should not substitute alternative products without checking with us, as formulations, dosages and quality may vary. It is very important that these are only used by you according to the instructions on the label, or ours, if we provide different instructions. Some products can be dangerous if consumed in excessive quantity, so if you are in any way unclear about dosage or duration of the therapy, please contact your practitioner.

We may receive a commission on such products or services and be informed that you bought them if you buy from a recommended supplier and tell the supplier that we referred you, but it is your choice whether you wish to buy from recommended suppliers, and also, whether you disclose that we referred you.

Advice on your own supplements
The fees agreed for consultations do not include review of products you are already taking or are considering independently of our recommendations. This is because unfamiliar products can take considerable time to evaluate, even when the precise ingredients list is readily available. This service is by arrangement with your practitioner, and will carry a minimum fee of £5 per product.

 

Assumption of Risk

You should recognise that specific foods, food supplements and/or herbal remedies may create allergic and possible anaphylactic reactions. You must therefore specify any food allergies/ sensitivities you are aware of on your health questionnaire, as well as any medications you or whoever you are responsible for are taking. We make no guarantees regarding the results achievable from any health plan. Results are individual and may vary.

Recordings

No audio or visual recordings may be taken of consultations without the written consent of all parties present. This applies both to in-person appointments and virtual appointments by videoconference or telephone.

Payment

All time spent is chargeable, whether in consultation, in telephone follow-ups, in analysing results or preparation of health plans. We will usually give a guideline cost for consultation, potential testing, supplements and herbs. If you are concerned about the cost, please discuss this at the outset. At the end of each consultation, you will be asked to pay for it. In some circumstances, you will be asked to pay in advance, in order to confirm a booking.

Where you arrange an appointment for yourself or someone else, you are solely responsible for these fees, unless clearly agreed otherwise. If another party pays for such fees, you agree that they are doing so on your behalf, not as principal, and you make clear to them that they have no legal recourse to us.

For physical and virtual consultations, we prefer bank transfer, but other options may be available.  All payment must be made at the time of the consultation. We do not give credit.

Cancellation

We will contact you with an email reminder of your appointment five to seven days prior to your appointment. If you wish to make a change or cancellation, you must do it quickly. We usually run waiting lists and your practitioner sets aside their time for you –so it is not fair on other clients if they have to wait, or on practitioners who lose time and money because an appointment goes to waste.

Therefore, a 50% cancellation charge may be applied by your practitioner if you do not give us at least one full clear business day before the appointment so we can offer the appointment to someone else. This means that cancellation of an appointment on a Monday must be notified to us by the previous Thursday.

And a 100% cancellation charge may be applied if you do not give us 24h notice of a cancellation, or do not turn up.

Arrival

Please arrive or be ready for a virtual consultation at least 5-10 minutes early. We operate on tight schedules and late arrival may result in a reduced consultation time. We will attempt to send you a SMS reminder before the consultation.

People you are responsible for

Please accompany or be present during consultations with all minors under 18 and other people you are responsible for. Specifically, young people under 18 must have a parent or guardian give consent to the treatment and sign forms if necessary. We reserve the right to decline treatment if a parent, carer or guardian is not available.

Data Protection

These terms should be viewed in conjunction with our privacy policy.

The personal information you submit to your Practitioner and we collect from you in health consultations is very sensitive and we take this responsibility very seriously. The lawful purpose for which we collect and process this data is for the provision of health care on the basis of a contract with a health professional.

You are entering a contract with Practitioner Joanne Wise of Intouch Chirocare Ltd (trading as Wise Nutrition) which acts as Data Controller in respect of your private information. These are our data retention policies:

  • We keep data for a minimum of 8 years from the last consultation for adults.

  • Where the client is a child under 16, we will store the data at least until their 25th birthday, and if a child reaches the age of 17 while under our care, then we will retain the records until at least their 26th birthday.

  • We may destroy or archive data after this time.

You have the right to complain to the Information Commissioner’s Office if you are unhappy with our data practices.
